Effect of Sublingual Nifedipine on Left Ventricular Systolic and Diastolic Functions during Isometric Exercise in Patients with Dilated Cardiomyopathy

Seventeen patients with dilated cardiomyopathy (DCM) and 11 normal subjects were studied by digitized M-mode echocardiography during isometric exercise to assess the effect of nifedipine on left ventricular systolic and diastolic functions. Following sublingual nifedipine (10 mg), blood pressure and left ventricular end-systolic dimension (Ds) were decreased in patients with DCM. Although peak-dD/dt/D did not change, peak+dD/dt/D, one of the left ventricular filling indices, improved during exercise after nifedipine. In conclusion, sublingual nifedipine showed a favorable effect on diastolic function but not on systolic function during isometric exercise in patients with DCM.
