Myxoma of the mitral valve is extremely rare. A mass was demonstrated on the anterior leaflet of the mitral valve in a 24-year-old male by two-dimensional echocardiography. Myxoma of the mitral valve leaflet was diagnosed by postoperative pathological examination. The patient had cerebral embolism twice before admission. An echocardiogram showed a round echodense mass resembling vegetation attached to the anterior leaflet of the mitral valve. The mitral valve was excised and replaced with a Björk-Shiley prosthesis. A hemispheric cauliflower-shaped (9 mm in diameter), non-pedunculated mass was observed in the center of the anterior mitral leaflet. A microscopic examination revealed that myxoid matrix and myxoma-like cells proliferated around the axis of collagen fibers. A review of the literature of mitral valve myxoma diagnosed by echocardiography was made.
